If you are not a Wilms Tumor patient and would like to participate please go to the second half of the page for Control Consent Registration Form. The last name of your child as recorded by the COG (Children's Oncology Group). 2. Your child's month and year of birth. 3. Your unique ID number (XX-XXXX) which is provided in your letter of invitation as well as in all of your study materials. We ask for these pieces of information in order to confirm your identity and maintain confidentiality. If you are a Wilms Tumor patient and would like to participate, please click at the top of the page Case Consent Registration Form. Your child's Last Name as indicated on his/her Birth Certificate. 3. Your Relationship to the child. 4. Your unique Screening ID number which is provided in your letter of invitation (if you were mailed one).
